Overview
In 2011, the South African National Department of Health (NDOH) developed the Integrated Chronic Disease Management (ICDM) model as part of its Primary Health Care (PHC) re-engineering. The pilot study of the model is ongoing in selected PHC facilities in Gauteng, North West and Mpumalanga Provinces. All the PHC facilities in Agincourt sub-district are included in this pilot.The study hopes to evaluate the effectiveness of the ICDM model in bringing about desired changes such as client satisfaction with quality of services, improvement in client morbidity status (reduced blood pressure/glucose levels and increasing CD4 count), and improved health service delivery (healthcare utilization and waiting time).
